New vendors will now be required to complete the Vendor Information Form and IRS Form W9. The vendor package should include both of these forms in addition to the ePayable Enrollment Form, ACH Enrollment Form and the SSN Disclosure Statement if the new vendor is an individual, sole proprietor or independent contractor.
IMPORTANT: Requesting departments should instruct the vendor that completed forms and documents should be submitted to the appropriate Appalachian State University department via fax or U.S. mail. Forms will not be accepted via email.
